Card company isn't resolving a dispute about a purchase or transfer. This is a prepaid award card issued by XXXX XXXX and administered by North Lane Technologies. Sales commissions are put onto a debit card. My account number was stolen and used multiple times between XX/XX/XXXX - XX/XX/XXXX. I contacted the bank multiple times by phone and email to resolve the issue over the last three months. It was so difficult because somehow they didn't have my correct address. I had to prove that it was my account. Finally, I was sent a fraud form on XX/XX/XXXX which I completed and tried to fax it 9 times over 5 days and it would not go through. They said that I had to either fax it or mail it ( I could not scan and email it ). I mailed it on XXXX. I kept calling to see if they received it. The
