I am writing to dispute the charge-off account from XXXX reported on my credit file, in accordance with the Fair Credit Reporting Act ( FCRA ), 15 U.S.C. 1681 et seq. The account information is as follows : Account Name : XXXX Account Number : XXXX Date of Charge-Off : XXXX XXXX I dispute this account because XXXX never sent me a XXXX form, which is required by the IRS to report canceled debt XXXX When I requested a XXXX form from XXXX, they stated that they do not send them out. I believe this is a violation of IRS regulations and the FCRA, which requires that credit bureaus ensure the accuracy and completeness of the information reported. This account was charged off on the following date XXXX XXXX. Since then, the account has repeatedly been
