Dispute of Bankruptcy Due to Identity Theft ( FCRA 605 ( b ) ) Dear All Credit Reporting Agencies, I am writing to formally dispute the inclusion of a bankruptcy on my credit report. This bankruptcy was filed fraudulently in my name without my knowledge or consent and is a result of identity theft. Under Section 605 ( b ) of the Fair Credit Reporting Act ( FCRA ), I am requesting its immediate removal from my credit report. I have enclosed the following documentation : XXXX. Identity Theft Report : A copy of the FTC Identity Theft Affidavit and/or a police report detailing the fraudulent activity. XXXX. Proof of Identity : Copies of my government-issued photo identification and a utility bill or bank statement as proof of residency. Under
