Their investigation did not fix an error on your report. I am submitting this formal complaint regarding JPMCB/Chase Banks failure to provide me with a XXXX form for tax reporting purposes. My previous complaint with the CFPB was closed on XX/XX/year> ( XXXX XXXX. XXXX ) based on JPMCB/Chase Banks response. However, the issue remains unresolved as I have not received the required XXXX form that was owed to me. As of today, I have not received any communication from JPMCB/Chase Bank that fulfills their legal obligation to provide this XXXX form.
