Credit reporting or other personal consumer reports — EQUIFAX, INC. (NY)

Information is missing that should be on the report. I for years have had a XXXX charge off tied to my credit which has negatively impacted my credit and ability to get loans. The charge off has an address of XXXX MA in which I never lived or received notice of this due to it being in XXXX and I live in XXXX. I am requesting this charge off which I have tried to get removed on multiple occasions and they just say its verified and the XXXX XXXX XXXX in Massachusetts removed as well. This is in direct violation of my consumer rights and will seek legal action if not removed
