I am writing to formally dispute inaccuracies on my credit report that are having a significant negative impact on my financial well-being and my ability to provide for my family. The information below on my report I have identified as identity theft, and this item is not related to any transactions by me at all. I was not able to move because of my credit profile being inaccurate due to these unknown accounts being opened in my name.
