Information belongs to someone else. Hello, I am writing this dispute for the following reasons : My personal information, such as my Social Security number, my driver 's license, and other personal information, was exposed in a data breach at a hospital in XXXX, Florida, the city where I live and the hospital where I go when I have health problems. Because of this, my information was used to open an account with XXXX XXXX XXXX with account number XXXX. This account is completely fraudulent because it was not opened with my consent. I want it removed from my credit history. The account is closed, but I still want it removed because it is not mine, nor did I open it. I have already reported it to the Federal Trade Commission ( FTC ) and am sending the report below. Please remove this accoun
