Credit reporting or other personal consumer reports — EQUIFAX, INC. (AZ)

Account information incorrect. Im writing because there is an account showing up on my credit report that is completely unfamiliar to me. I have no knowledge of this account, never opened it, never authorized it, and have no connection to it whatsoever. Simply put it does not belong to me. This appears to be a case of mixed or misreported credit data, and its not just frustrating its damaging. I work hard to maintain my credit, and having someone elses account falsely tied to my name is completely unacceptable. I want to be absolutely clear : this is not my debt, not my account, and not my responsibility.
