Account information incorrect. In either XX/XX/XXXX or XX/XX/XXXX I applied for a credit card with XXXX XXXX Account # XXXX . I then had a medical issue and went to the hospital followed by an extended stay for XXXX . In XX/XX/XXXX I was made aware by XXXX XXXX that my account was past due {$130.00}. These charges were for a {$75.00} annual fee followed by late fees. I told the representative that I was in the hospital and # 1 NEVER RECEIVED THE CARD AND # 2 NEVER A CTIVATED THE CARD. When I questioned the representative as to how they could charge me without ever activating the card the representative agreed that this was a mistake and apologized and told me not to worry about any charges, that my account with the company would be closed and balance brought to {$
