None. On XX/XX/XXXX, I wrote to LexisNexis Consumer Center and requested a security freeze be place on my Consumer Disclosure report. I also requested to opt out of LexisNexis disclosing future information until further notice including the C.L.U.E report and other reports furnished LexisNexis regarding all personal information of mine to credit bureaus and creditors, but also future potential employers until I lift this restriction. Finally, I requested LexisNexis to send me a FREE copy of the information you are reporting about me as of the date you receive this request. In my request, I provided my Driver 's License Number and Social Security Number as well as my Name, full date of Birth and Address, and also enclosed a copy of my State issued Driver 's license and Social Security Card.
