Debt collection — The Outsource Group, Inc (UT)

Debt is not yours. XX/XX/XXXX - Went to XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX which I denied having, and I never once entered the facility, never checked-in, never tested, etc. XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX a letter to me claiming that my " request '' for financial assistance was denied for this service that never occurred. I never had a test nor a request for financial assistance. There are no lab records -- nothing. I phoned them and they said they would get back to me after investigation. They didn't.
