Information belongs to someone else. AFFIRM CASE ID # XXXX My personal information was used without my permission and used to open AFFIRM loan account and is now in collections. I discovered this fraud and emailed AFFIRM the requested affidavit fraud form. They have not responded within the promised XXXX hour time frame. The account has caused my fico scores to dip significantly and I am requesting for AFFIRM to immediately rectify the error and delete the fraud accounts from XXXX and XXXX XXXX and XXXX.
