Reporting company used your report improperly. Thank you for your efforts in removing some of the items from my file related to alleged fraudulent activity. However, there are still items that need to be addressed. According to FCRA Section 605, you are required to remove any items listed in ID Theft Report # XXXX. Per Section 605B ( a ) of the Fair Credit Reporting Act, you must remove any items opened without my knowledge, consent, or authority within four business days upon receipt of the following : ( 1 ) Appropriate proof of my identity ; ( 2 ) A copy of the identity theft report ; ( 3 ) Identification of the disputed information by me ; and ( 4 ) A statement by me affirming that the information is not related to any transaction I conducted.
